The Corporate Division of Health Information Management supports the management and integrity of patient medical information through a comprehensive electronic health record system.
How do I obtain a copy of my health information/medical records?
Patients are encouraged to request health information online using the MyUHealthChart patient portal. Information electronically downloaded through the portal is provided at no cost. Records are available electronically for services received after May 2010 with the exception of entire medical records for services provided at University of Miami Hospital and Clinics inpatient units.
Records not available through the patient portal can be obtained by contacting the appropriate facility. Please download and complete the appropriate release form; fax, mail or contact the appropriate Health Information Management (Medical Records) department, see below.
What is MyUHealthChart?
MyUHealthChart gives you the ability to access your health information at your convenience, through a secure patient portal website. MyUHealthChart allows you to be actively involved in your health care, the ability to communicate with your providers, and to help you stay informed. Click here to access MyUHealthChart. You can now access your MyUHealthChart from your iPhone/iPad or Android device by downloading the MyChart app from the App Store or Google Play. After you download the MyChart app, please select the UHealth provider.
Is there a charge associated with my request for copies?
Information electronically downloaded through the patient portal is provided at no cost. Healthcare facilities and patients are not charged for copies of records if requested for continuity of care. Fees are less if records are generated in electronic format vs. paper. All other fees for medical information comply with State of Florida statutes and applicable federal regulations.
How long will it take for requests to be processed?
Expected turn around times for electronic requests is 4 business days and most records are available within 48 hours. Please note only finalized documentation is available for release and may contribute to any unforeseen delays.
